Additional Fees for First Visits Without a Referral

Medical: ¥11,000 (including tax)
Dental: ¥11,000 (including tax)
The above fees have been in effect since November 1, 2020.
Fees for patients without referrals are subject to tax.

Under the April 2016 amendment to the Health Insurance Law, community medical support hospitals of 200 beds or more are required to collect least ¥5,000 yen in the form of “an additional fee for patients without referrals” from first-time patients who lack referrals from other medical facilities.
As part of this system, the Center collects the above-mentioned fee from both out- and in-patients.

Additional Fees for Return Visits Without a Referral

Medical: ¥11,000 (including tax)
Dental: ¥11,000 (including tax)
The above fees have been in effect since November 1, 2020.
Fees for patients without referrals are subject to tax.

Under the April 2016 amendment to the Health Insurance Law, community medical support hospitals of 200 beds or more are required to provide referrals to patients who have completed specialist or emergency treatment and who are now in stable condition.
If the patient returns to the same community medical support hospital for consultation without a referral after having been referred to a different community medical support hospital of 200 beds or more, the hospital is further required to collect least ¥2,500 yen in the form of “an additional fee for patients without referrals”.
As part of this system, the Center collects the above-mentioned fee from both out- and in-patients.

The fee is not collected if:

1. the patient has a referral from another medical facility (osteopathic, orthopedic, and acupuncture and moxibustion clinics and foreign medical facilities excepted),
2. a request has been received for a detailed examination following diagnosis for cancer or another specified medical condition,
3. the patient is admitted directly following consultation,
4. the patient was referred from the medical to the dental department or vice versa,
5. the patient is entitled to medical assistance under the Public Assistance Act,
6. the patient is receiving public assistance for a specified disease, disability, or other condition (assistance for medical treatment of infants, children of compulsory school age, and members of single-parent families excepted),
7. the treatment is part of a clinical trial,
8. the patient is a disaster victim, or
9. the consultation is at the patient’s expense or for injuries received in the line of duty or as the result of a workplace or traffic accident.

From January 1, 2021, the fee is collected from patients currently receiving treatment from (or who have made a reservation with) another department at the Center.

Additional Fees for After-Hours Visits

Fee for after-hours visits: ¥11,000 (tax included)

The above fee has been in effect since November 1, 2020.
As of September 2020, children of junior high school age or younger are not subject to additional fees for after-hours visits.
Fees for after-hours visits are subject to tax.

The Tokyo Municipal Government has designated the Center a “lifesaving center for emergency patients with life-threatening illnesses or serious conditions requiring treatment involving multiple medical specialties” and a “general lifesaving medical center for the treatment of pregnant mothers and the perinatal care of mothers and fetuses that will without fail accept pregnant women requiring emergency lifesaving treatment”.
The Center therefore offers 24-hour emergency medical support for gravely-ill patients requiring hospitalization. In order that it can provide such support, the Center has received permission to collect additional payment from patients with mild, non-urgent medical conditions in the form of “fees for after-hours medical treatment from facilities and practitioners subject to the rules of the health insurance system”.
The above-mentioned fee is collected from patients by the Center as part of this system.

The fee is not collected from patients who:

1. are junior high school age or younger,
2. have been referred for emergency outpatient consultation,
3. are admitted directly following consultation,
4. have undergone an abrupt change in condition during treatment at the Center,
5. have made reservations to give birth at the Center and are now being admitted for obstetric emergencies,
6. have been admitted at the direction of a doctor at the Center,
7. are entitled to medical assistance under the Public Assistance Act,
8. are receiving public assistance for a specified disease, disability, or other condition (assistance for medical treatment of infants, children of compulsory school age, and members of single-parent families excepted), or
9. have been injured in the line of duty or as the result of a workplace or traffic accident.
